Taking naproxen with an oral corticosteroid can add to gastrointestinal bleeding risk. Review the exact steroid, overlap, and personal risk factors before continuing both.
Why the combination deserves review
Oral corticosteroids and NSAIDs can each be used for inflammatory conditions, but the naproxen prescription label identifies concomitant oral corticosteroid use as a factor that increases gastrointestinal bleeding, ulceration, and perforation risk. The fact that both medicines were prescribed does not prove that the combination is safe for every duration or patient.
Clarify which steroid is involved
“Steroid” can refer to an oral tablet, injection, inhaler, nasal spray, skin product, or other formulation. The prescriber or pharmacist needs the exact name, route, amount, start date, and planned duration. Prednisone, methylprednisolone, dexamethasone, and local steroid products do not create identical exposure.
Factors that raise concern further
- Previous ulcer or gastrointestinal bleeding.
- Older age, poor health, smoking, or heavy alcohol use.
- Aspirin, an anticoagulant, antiplatelet medicine, SSRI, or SNRI.
- Longer naproxen exposure or more than one NSAID.
- Advanced liver disease or a clotting disorder.
Symptoms to act on
Emergency evaluation is appropriate for bloody vomit, black tar-like stool, fainting, or heavy bleeding. Prompt clinical advice is warranted for persistent stomach pain, worsening heartburn, unusual bruising, weakness, or symptoms of anemia. Do not simply add an OTC acid reducer without reviewing the full regimen.

- Are both medicines necessary at the same time?
- What is the shortest intended overlap?
- Does the ulcer/bleeding history require an alternative?
- Is prescribed gastroprotection indicated?
- Who should be contacted if symptoms develop, and should either medicine be held?

Do inhaled steroids create the same risk as prednisone?
Systemic exposure differs by product and route. Provide the exact medicine and route rather than assuming every steroid carries identical risk.
Can I separate the medicines by a few hours?
Spacing does not necessarily remove their overlapping gastrointestinal risk. The need for both medicines and the duration of overlap still require review.
What if both were prescribed by different clinicians?
Contact the pharmacy or prescribing teams so one clinician reviews the complete regimen, including OTC naproxen.
